RTOG 0630 Requirements

 

Credentialing is required before patients can be entered onto this study. The credentialing requirements are different depending on whether you are using a 3D-CRT or IMRT technique. You can be credentialed for either technique or for both depending on your physician's treatment preference. All participants must be credentialed for IGRT.

In order to complete the 3D-CRT credentialing process, the following items must be completed:

• Complete and submit the 3D-CRT and IGRT Facility Questionnaires to RTOG.

• A successful digital 3D-CRT treatment planning data submission test to the ATC.

• Submit a previously treated anonymized sarcoma IGRT patient’s data to the ATC.

In order to complete the IMRT credentialing process, the following items must be completed:

•  Complete and submit the IMRT and IGRT Facility Questionnaires to RTOG.

• Irradiate the IROC Houston's IMRT H&N phantom or pelvic phantom. Please fill in the request form online to request a phantom.

•  Submit your phantom plan digitally to the ATC.

•  Submit a previously treated anonymized sarcoma IGRT patient’s data to the ATC.

Note:

Irradiation of the pelvic phantom satisfies part of the credentialing requirements for protocol 0630 but does not necessarily satisfy the IMRT phantom irradiation requirements for other protocols.
Institutions that are credentialed to participate in other IMRT protocols requiring a phantom irradiation need not repeat the phantom irradiation for credentialing for this protocol. Institutions that are credentialed to participate in other IMRT protocols should fill in the Credentialing Status Inquiry Form.
