Credentialing is required before patients can be entered onto this study.
In order to complete the credentialing process, the following items must be completed:

• Complete and submit all relevant parts of the Facility Questionnaires to RTOG.

• Successfully complete the IGRT credentialing requirements. Details can be found on the ATC website.

• Irradiate the RPC's spine phantom. Please fill in the request form online to request a phantom.

•  Submit your phantom plan digitally to the ATC.

Note:

Previous phantom irradiations will not satisfy the phantom requirement for this protocol.
