RTOG 0815 Requirements

 

Please fill out the credentialing status inquiry form to let us know that you would like to be credentialed for this protocol.

This study requires credentialing before patients can be entered onto this study. The credentialing requirements are different depending on whether you are using a 3DCRT or IMRT technique. Also, you must complete separate credentialing requirements if you wish to use one or both of the brachytherapy boost techniques. You can be credentialed for either 3DCRT or IMRT or both depending on your physician’s preference. Likewise, you can be credentialed for either LDR or HDR, both or neither brachytherapy boost modalities.

In order to complete the 3D credentialing process, the following items must be completed:

• All participants are asked to update or complete a new IROC Facility Questionnaires

•  This protocol requires each institution to submit electronic patient data to RTOG using TRIAD. For instructions click here for TRIAD. Please note, with CTSU's intergration with TRIAD, IRB approval is required in order to submit credentialing data.

In order to complete the IMRT credentialing process the following items must be completed:

• All participants are asked to update or complete a new IROC Facility Questionnaires

•  This protocol requires each institution to submit electronic patient data to RTOG using TRIAD. For instructions click here for TRIAD. Please note, with CTSU's intergration with TRIAD, IRB approval is required in order to submit credentialing data.

• The institution must irradiate the IROC Houston's IMRT H&N phantom.  Please fill in the request form online.

Note:

•  If your institution previously completed a pelvic phantom irradiation, that irradiation may satisfy the credentialing requirements for this protocol.

• Institutions credentialed for IMRT on a previous RTOG protocol are eligible to participate on this protocol using IMRT without further IMRT credentialing.

LDR Brachytherapy Credentialing:

Institutions wishing to enter patients onto protocols that include permanent prostate brachytherapy implants must be credentialed prior to participation in the study. Because these procedures require a team effort, the institution, the radiation oncologist and radiation physicist are credentialed as a team.

  1. LDR Facility Questionnaire
  2. LDR Prostate Knowledge Assessment Form

    • RTOG requires that the prostate team must have performed at least 10 TRUS guided prostate implants. At the bottom of the Knowledge Assessment Form you will be asked to attest to this by your signature.

  3. Reference Case 1
  4. Hardcopy of one recent implant case performed by the radiation oncologist and physicist team

    • Ultrasound images, needle loading information, DVH and isodoses in either a pre-plan or real-time plan format

    • Post implant treatment plan

  5. Electronically submit post implant data for the same patient to the TRIAD. Directions on submitting data can be found on the TRIAD webpage. A list of items that need to be submitted to the TRIAD are can be found on the TRIAD webpage.
  6. If performing TRUS-based planning, please complete the TRUS Questionnaire.

Note:

An institution that has been previously credentialed for a radiation source model does not need to submit the complete package to become credentialed for an additional source model. It is only necessary to submit the two reference cases, performed with the new source model. Similarly, changing to a different treatment planning system requires re-credentialing, and only the two reference cases need to be submitted. However, if there is a change in physicist, you need to complete items 1 and 2 above. A change in physician requires a submission of items 1, 2, and 4.

HDR Brachytherapy Credentialing:

Institutions wishing to enter patients onto protocols that include HDR prostate brachytherapy implants must be credentialed prior to participation in the study. Because these procedures require a team effort, the institution, the radiation oncologist and radiation physicist are credentialed as a team.

  1. HDR Prostate Facility Questionnaire (including benchmark cases)
  2. HDR Prostate QA Knowledge Assessment Form
  3. Data for a recent HDR prostate patient to be submitted digitally to RTOG using TRIAD.
  4. If performing TRUS-based planning, please complete the TRUS Questionnaire.

Note:

Institutions previously credentialed for RTOG 0321 are eligible to participate on this protocol using HDR if there has not been a change in physician or physicist. Please complete the HDR Knowledge Assessment Form if you were credentialed for 0321, 0924, 1115 and you wish to be credentialed for 0815.
