RTOG 0831 Requirements

 

This study requires credentialing before patients can be entered onto this study. The credentialing requirements are different depending on whether you are using a 3DCRT or IMRT technique. You can be credentialed for either technique or for both depending on your physician's treatment preference.

In order to complete the 3D credentialing process, the following items must be completed:

• All participants are asked to complete and fax/e-mail the Facility Questionnaires to RTOG at 215-940-8817 or rtog-facquest@phila.acr.org. Questions about the questionnaires can be directed to Tammy McGlade at 215-574-3219.

• Submit an actual patient treatment plan digitally to the ATC.

 

In order to complete the IMRT credentialing process, the following items must be completed:

• Complete and submit the Facility Questionnaires to RTOG.

• Submit an actual patient treatment plan digitally to the ATC.

• Irradiate the IROC Houston's IMRT H&N phantom or pelvic phantom.  Please fill in the request form online.

Note:

• Irradiation of the pelvic phantom satisfies part of the credentialing requirements for protocol 0831 but does not necessarily satisfy for IMRT phantom irradiation requirements for other protocols.

• Institutions that are credentialed to participate in other RTOG IMRT protocols need to fill in the credentialing status inquiry form to verify their ability to enter patients onto this protocol.

Brachytherapy Credentialing:

Institutions wishing to enter patients onto protocols that include permanent prostate brachytherapy implants must be credentialed prior to participation in the study. Because these procedures require a team effort, the institution, the radiation oncologist and radiation physicist are credentialed as a team.

The treatment team is required to submit five items.

  1. Facility Questionnaire
  2. Knowledge Assessment Form
  3. RTOG requires that the prostate team must have performed at least 10 TRUS guided prostate implants. At the bottom of the ATC Knowledge Assessment Form you will be asked to attest to this by your signature.
  4. Reference Cases 1 and 2
  5. Hardcopy of one recent implant case performed by the radiation oncologist and physicist team
  6. Ultrasound images, needle loading information, DVH and isodoses in either a pre-plan or real-time plan format
  7. Post implant treatment plan
  8. Electronically submit post implant data for the same patient to the ITC. Directions on submitting data can be found on the ATC webpage. A list of items that need to be submitted to the ITC are can be found on the ATC webpage.

Note:

An institution that has been previously credentialed for a radiation source model does not need to submit the complete package to become credentialed for an additional source model. It is only necessary to submit the two reference cases, performed with the new source model. Similarly, changing to a different treatment planning system requires re-credentialing, and only the two reference cases need to be submitted. However, if there is a change in physicist, you need to complete items 2 and 3 above. A change in physician requires a submission of items 2, 4, and 5.
