For this study, IMRT is mandatory, and IGRT is optional. (IGRT is mandatory when using reduced margins)

If an institution decides to use IGRT, that institution must be credentialed both for IMRT and for head and neck image-guided radiation therapy(IGRT).

In order to complete the IMRT credentialing process the following items must be completed:

• All participants are asked to complete and fax/e-mail the Facility Questionnaires to RTOG at 215-940-8817 or rtog-facquest@phila.acr.org. Questions about the questionnaires can be directed to Tammy McGlade at 215-574-3219.

• Submit an actual patient treatment plan digitally to the ATC.

• Irradiate the RPC's IMRT H&N phantom.  Please fill in the request form online.

In order to complete the IGRT credentialing process, the institution must complete the additional requirements detailed on the ATC website.

Note:

Institutions that are credentialed to participate in other RTOG IMRT protocols requiring an IMRT H&N phantom irradiation need not repeat the phantom irradiation for credentialing for this protocol.
