This protocol requires each institution to complete a benchmark for credentialing, whether they are using 3D-CRT or IMRT treatment modalities. Create plan per protocol (see guidelines). Submit the benchmark plan electronically and a completed DDSI form to TRIAD for review. Please note, with CTSU's integration with TRIAD, IRB approval is required in order to submit credentialing data.

If you have not already completed the 3D credentialing process for this body site (e.g. RTOG 0126), the following items must be completed:
