Institutions that were previously credentialed to participate in another protocol or have a question about your status for this protocol, please fill out the credentialing status inquiry form.

In order to complete the 3D credentialing process the following 2 items must be completed:

• All participants are asked to update or complete a new IROC Facility Questionnaires

• Submit an actual patient treatment plan digitally to the TRIAD.

In order to complete the IMRT credentialing process, the following 3 items must be completed:

• All participants are asked to update or complete a new IROC Facility Questionnaires

• Create a TRIAD account with RTOG for patient data submission. See section 12.2 of the protocol for directions.

• Irradiate the IROC Houston's IMRT phantom. Please fill in the request form online.

Note:

Institutions that were previously credentialed to participate in another RTOG protocol where phantom irradiation was requuired for the credentialing process or have a question about your status for this protocol, please fill out the credentialing status inquiry form.

Acceptable treatment planning system(TPS) / algorithm

When institutions change technology, it is necessary to repeat the credentialing process, unless an institution is credentialed for IMRT and wishes to use a non-IMRT photon treatment delivery. Fundamental changes in the techniques used for motion management require repeat credentialing as well.
