Institutions that were previously credentialed to participate in another protocol will be notified if any additional credentialing steps are required.

For this study, IMRT is mandatory, and IGRT credentialing is mandatory when using PTV margins < 5 mm.

If an institution uses IGRT, that institution must be credentialed both for IMRT and for head and neck image-guided radiation therapy(IGRT).

This protocol requires each institution to submit electronic patient data to RTOG using TRIAD. For instructions on TRIAD, click here. Please note, with CTSU integration with TRIAD, IRB approval is required in order to submit credentialing data.

In order to complete the IMRT credentialing process the following items must be completed:

• All participants are asked to update or complete a new IROC Facility Questionnaires

• Submit an actual patient treatment plan digitally to TRIAD.

• Irradiate the IROC Houston's IMRT H&N phantom.  Please fill in the request form online.

In order to complete the IGRT credentialing process, the institution must complete the additional requirements detailed on the TRIAD website.

Note:

Institutions that are credentialed to participate in other RTOG IMRT protocols requiring an IMRT H&N phantom irradiation need not repeat the phantom irradiation for credentialing for this protocol.
