Institutions that were previously credentialed to participate in another protocol will be notified if any additional credentialing steps are required.

This protocol requires each institution to submit electronic patient data to RTOG using TRIAD. For instructions on TRIAD, click here. Please note, with CTSU integration with TRIAD, IRB approval is required in order to submit credentialing data.

Credentialing is required before patients can be entered onto this study. In order to complete the credentialing process, the following items must be completed:

For institutions planning to use reduced margins (less than 5 mm), successfully complete the IGRT credentialing requirements. Details can be found on the RTOG 1205 RTQA Credentialing Page.

In order to complete the 3D credentialing process, the following 2 items must be completed:

• All participants are asked to complete and submit the IROC Houston Facility Questionnaire.

•  Submit an actual patient treatment plan digitally to TRIAD.

Credentialing for IMRT Photon Therapy Treatment, the following 3 items must be completed:

• All participants are asked to complete and submit the IROC Houston Facility Questionnaire.

• Submit an actual patient treatment plan digitally to TRIAD.

• Irradiate the IROC Houston's IMRT H&N phantom. Please fill in the request form online.

Note:

Institutions that are credentialed to participate in other RTOG IMRT protocols requiring an IMRT H & N phantom irradiation need not repeat the phantom irradiation for credentialing for this protocol.When institutions change technology, it is necessary to repeat the credentialing process. Also fundamental changes in the techniques used for IGRT or motion management require repeat credentialing. 

Credentialing for Proton Therapy Treatment:

• Successful completion of the IROC Houston proton head phantom irradiation and review.

• Complete and submit a IROC Houston Facility Questionnaire.

• Successful completion of a site visit by the IROC Houston.
